Research about the usage changes as to the antimicrobial restriction policy

Abstract
Effective antimicrobial agents are an essential part of patients¢¥s care. A use of antimicrobial agents is associated with development of resistant strains and excessive use increase costs unnecessarily.
Therefore various strategies, such as restrict the use of antimicrobials, have been devised to control antimicrobial agent for infection has been enforced from July, 1992. As evaluation of this policy had never been carried out, an effort to evaluate this policy and to suggest the most effective policy was begun. From 1988 to 1994, total amount of used antimicrobials, isolated microorganism number and patient number were analyzed to evaluate the influence of this policy. And occupation rate of second and restricted antimicrobials, increasing rte of antimicrobial resistance rate and cancer patient number were analyzed. As a result, for recent 3 years (1991-1994), enforcement of an antimicrobial restriction policy led to decrease the ascending rate of total antimicrobial amount used(9.3%), compared to that of patient number(20.4%) and isolated microorganism number(41.5%). In spite of restriction, occupation rate of second and restricted antimicrobials have been increased. It might be due to increasing rate of immunodeficient cancer patient and antimicrobial resistant rate. And the trend to select the newer antimicrobial by physician was also one of the most important factor. A policy to restrict the use of antimicrobial agents in hospital require not only limitation of the antimicrobial amount but periodic review and revision in response to change in the practice of infection disease and patient character. And as new patients of antimicrobial use emerged, the restriction policy must be modified as necessary.
